Because each treatment or procedure is unique, insurance coverage qualifications may differ. Please contact your insurance company directly to ensure that we are a part of your specific plan and to inquire about your policy. Please keep in mind that your insurance dictates your medical benefits, so knowing your coverage is your duty. If you have an insurance plan for which we are not a contracted provider, we will gladly provide care; however, the complete cost of your visit will be your responsibility at the time of service. We do have reduced and competitive out of pocket prices for patients without insurance coverage.
Referrals
Some insurance plans may require a primary care physician (PCP) evaluation before seeing a dermatologist or other specialist. If your plan needs PCP authorization, you must get a referral order before scheduling your appointment. If your insurance needs a reference, please provide correct documentation at the time of your visit. If you or your PCP do not present the necessary documentation by the scheduled appointment time, you must pay for your visit in full or reschedule.
Copayments, Coinsurance & Deductibles
A copayment is a fixed financial amount that you must pay for each office visit. If a procedure is conducted, all claims are subject to a deductible (i.e., biopsy, cryosurgery, excisions, etc.). A deductible is the amount you must pay before your insurance provider begins paying for your medical expenses. In addition to your copayment or deductible, certain insurance plans may include coinsurance, which means you may be responsible for a part of healthcare expenditures. It is your obligation to comprehend and acknowledge the scope of your plan, as well as any associated copayment, deductible, or coinsurance. Should your insurance carrier advise us that further payment is due from you, you will be billed for these amounts.
Self-Pay
Patients who are responsible for their own medical bills must pay at the time of service. Prior to the appointment, the administrative staff will be unable to provide specific pricing. The provider will calculate your charge based on the suggested course of treatment, and payment is payable in full at the time of service.
Payment Options
Payment will be collected at the time of service, including copays, coinsurance, patient balances, and unmet deductibles. We accept cash, check, Visa, Master Card, and American Express as forms of payment. We also have a credit card on file (CCOF) policy to make transactions go more smoothly. Our third-party merchant service processor stores the credit card information responsibly and securely on a secure site. Because your credit card information will not be stored in our office’s electronic health record, there will be no financial information associated with your file.
At the Time of Your Office Visit, a Valid Photo ID and Your Insurance Card are Required.
Without a valid insurance card, we are unable to send a claim to your insurance company. If you don’t have a valid insurance card, you’ll have to pay for the visit when you receive the service or treatment.
Please let us know if your address, phone number, or insurance plan changes. You should also bring your current insurance card to each visit. If you fail to update us regarding changes to your insurance plan, your insurance claim could be turned down, and you will be required to pay for the visit or treatment out of pocket.
